NEW ERRA: TRANSPERINEAL ULTRASOUND GUIDED BIOPSY IN PREVIOUSLY NEGATIVE TRANSRECTAL ULTRASOUND GUIDED BIOPSY

Abstract


Background:Prostate carcinoma affects the prostate gland, located in the male reproductive system. Trans-rectal ultrasound (TRUS) is a common method used to diagnose prostate cancer. Trans- perineal ultrasound-guided biopsy involves inserting a biopsy needle through the perineum (the area between the scrotum and anus) and into the prostate gland under ultrasound guidance. This method is considered to be more accurate than TRUS because it allows for more precise targeting of suspicious areas of the prostate gland. Patients with negative TRUS results but a high suspicion of prostate cancer may benefit from a trans-perineal ultrasound-guided biopsy.Objective:

 

To determine the frequency of carcinoma of prostate through trans- perineal ultrasound-guided biopsy in previously negative trans- rectal ultrasound guided biopsies.

Study Design:

 

Descriptive Observational Study.

 

Setting:

 

The study was conducted at the Department of Radiology, SIUT, Karachi, Pakistan.

Duration:

From December 16, 2022 to to June 15, 2023.

 

Materials And Methods:

All patients underwent Transperineal ultrasound-guided biopsy. The prostate gland was evaluated thoroughly by TRUS from the base to apex to identify any suspicious areas and to determine the prostate volume (PV). The biopsy procedure was performed by using 18 G, 20 cm long biopsy needles. Multiple biopsy cores were obtained from each of the six biopsy regions depending on the size of the gland, extending from the apex towards the base of the gland.

Results:

The mean ± SD of age was 64.19±8.68 years. The PV of the patients ranged from 22 to 133 ml with a median of 43.3.The prostate density of the patients ranged from 0.08 to 2.83 ml with a median of 0.42 and with an interquartile range of 0.62 and C.I (0.51----0.65). Carcinoma of the prostate was found to be in 41 (31.8%) patients.

Conclusion:

A significant number of Carcinoma of the prostate missed on TRUS guided biopsies were picked by Transperineal biopsy.
