Abstract

Spinal epidermoid cysts are rare benign intradural extramedullary lesions, that can be either congenital or acquired after spinal interventions. It can produce variety of symptoms based on location and can be treated surgically but the preoperative diagnosis can be challenging because of unusual findings on MRI. Here we report a case of spinal epidermoid cyst identified based on MRI findings in an 18-year-old man, who presented with mild lower back pain and progressive lower limb weakness. MRI featured a cystic lesion at L2–L3, isointense on T1-weighted images with a small hyperintense focus, hyperintense on T2/STIR, and enhancement.
